the sentence means "你在想什么,你就会实现愿望" etc. in directly. But there are many derived meanings in chinese . You can find the meaning like these sentences bellow.

Maybe you need give an explanation in english first?
if the sentence means "all your dream will come true.",you can translate it as “心想事成”,“随心所愿。”or “心有多大,天有多高。”and so on.

if the sentence means "once an action appears in your mind,you will done it",you can translate it as "形随意动".
