Heto   Ito ang aso namin, si Chase ang pangalan niya. Brown  Kayumanggi (brown) at kulot ang balahibo niya at brown   kayumanggi rin ang mga mata niya. Malaking aso si Chase at mahabang  ang tenga   tainga niya. Madalas G gusto ni Chase sa park, (at ng mga patpat,) I don't understand what do  you want to say here pero ayaw niya ng  tali at ng mga ardilya (squirrels) at Mabait na mabait   sobrang bait si   ni Chase.

Ito si Chase, ang aso ko!  (Correct, but better decide to use either "namin" or "ko" for the title and your first sentence to be consistent.)

Heto ang aso namin, si Chase. (Correct) May also be “Heto ang aso namin na si Chase.”

Brown at kulot ang balahibo niya at brown din ang mga mata niya. (You need the “din” (also) there because the colors are the same.)

Malaking aso si Chase at mahaba ang tenga niya. = Chase is a big dog and his ears are long. (If you would like to use “mahabang tenga”, then the sentence would be, “Malaking aso si Chase at may mahabang tenga siya. = Chase is a big dog and he has long ears.) (The original spelling of “tenga” (ear) was “tainga”, but the other spelling came about because it is more representative of how we actually pronounce it.)

Gusto ni Chase sa park at (ng) mga patpat (? wooden stick), - (the “ng” may be omitted) (Translation: Chase likes going to/being in the park and sticks. Even in English, the pairing of park and sticks does not go that well. Did you mean something else for “patpat”?)

- pero ayaw niya SA/NG tali at SA mga squirrels. = but he does not like strings/rope (for binding) and squirrels.

- pero ayaw niyang (or “niya ang”) nakatali at SA mga squirrels. = but he does not like being on a leash and squirrels.

Mabait na mabait si Chase. (Correct, but the more common way to say it would be, “Napakabait ni Chase.”)
