"It freaks me out" is correct.  "It makes me feel weird"  is also correct.

I have never heard of "It weirds me out"  Unless you mean "It wears me out"?

They're both pretty much the same, but "weirds me out" is not as strong.  I hear it used quite often, but it's a slang term and isn't proper English.

"It freaks me out" - I would use that when I am unpleasantly surprised or startled by something... such as if I saw many rats coming out of a sewer. "It weirds me out" - would describe the feeling when something causes uneasiness or vague fear...such as entering a strange dark building at night
