It is correct and polite. "Could you please" is generally more polite and formal than "can you please".


Although "can" and "could" generally mean the same; "could you please" generally tells someone to do something; where "can you please" asks if they are able to do something. For example; if you ask someone, "could you please pass me the document?" - they will pass you the document. Where if you ask, "can you please pass me the document?" - they will reply yes or no; but may not actually pass you the document.

Yes, perfect. Instead of 'documents or information' you could possibly refer to it as 'documentation' which would cover both aspects......ie 'Could you please let me know if any other documentation is needed.' However, it is fine as it stands.
