    It would be more correct to say: Be careful about where you might have made a mistake.
    There are other variations.
    Use "where" because this expression is used when you might be checking something over, like an essay. You aren't concerned with when you made a mistake, but where.

    我觉得应该选B,表示在犯错误的地方应该更小心。when 的话貌似解释不通。
