    Başına hiç böyle bir şey geldi mi?
    Başına böyle bir şey hiç geldi mi?

    If you're talking with someone that you are not close with, or when you want to be formal you should say "başınıza"

    Or if you are asking this question to more than one person, you should again use "başınıza".


    That "z" suffix is actually the plural form of second person. But it's also used to make the sentence formal.

    Nasılsın? (I'm just talking with you.)
    Nasılsınız? (I'm talking with you and there are some others.)
    Nasılsınız? (I'm talking with just you, but because we are not close, I use this form.)

     

    Hiç başına böyle birşey geldi mi? (dont forget the question tag "mi")
    Hiç başınıza böyle birşey geldi mi?

    When you say "başına" , then you are adressing to 2nd-person singular pronoun.
    When you say "başınıza" , then you are adressing to 2nd-person plural pronoun.
