The direct translation should be "¿Cómo estás tú?", but as "estás" is conjugated on singular second person, then "tú" is redundant. So, "¿Cómo estás?" is the correct answer :P
¿Cómo estás? is the informal form, just like "tum kaise ho"
¿Cómo está? is the formal/polite way, like "aap kaise hain"
"Tu" means you in informal way, "Usted" means you in formal way.
Answer to this question would be "Estoy bien" equal for both genders.... :))))
