What is the best way to say, "I'm calling to check on you" Or "I was just stopping by to check on you" in Spanish?

Plsdeluno is right, it literally means "saber cómo estás?".
I would say "Te llamé para saber/ver como estas/estabas". The proper way is " Te llamé para SABER cómo ESTÁS" but where I live we usually say "... para VER como ESTABAS".

No soy nativo pero yo diría, te llamo para saber cómo estás, te llamaba para saber cómo estás. (por teléfono)
O si tocara la puerta de la casa de mi amigo, diría, estoy aquí para saber cómo estás.
