Which is the correct adverb placement?

It was late in the afternoon when the fire completely burned out.
or
It was late in the afternoon when the fire burned out completely.

Furthermore, I'd have one more question.

The sentence "She drives carefully" is correct- I am sure of that.
Now, which of two sentences are more acceptable:
"He opened the door carefully"
"He carefully opened the door".

You have asked a great question!

    1. He opened the door carefully.
    2. He carefully opened the door.

Some (repeat: some) people feel that "carefully" in No. 1 is absolutely necessary in order to express the writer's meaning. Of course, it depends on the context. That is, what were the sentences before and after this sentence?

Some people feel that "carefully" in No. 2 might be omitted (not used) and the sentence would still express the author's meaning. In other words, the adverb "carefully" is not essential to the meaning of the sentence. (But -- for some reason -- "carefully" is essential in No. 1.)

The placement of adverbs fascinates me, too. So I look forward to other answers.
