Girlfriend vs. Girl friend

I know that when someone writes "girlfriend" he means the girl he loves
and when he writes "girl friend" he means just a friend.
My question is: when someone is speaking, how do you know which one they mean?

To emphasise the difference, you would say "girl friend" as two distinct words. (or, use "female friend").

Also, in the USA, a male would never say refer to his female friends as "girl friends." As Peachey indicated, he would use "female friend," or perhaps "girl I'm friends with."
A female might refer to her female friends as "girlfriends." If she is a lesbian or if you are uncertain whether she is, I suppose this could be confusing, but usually the context of the statement makes it clear that she means just a friend and not a romantic partner.
