It's an expression that is often used when talking about a problem (social, technical, political, economic, etc.) that needs to be solved. It means that was has been done so far to address the problem has not been very helpful and that a new approach must be taken. It becomes necessary to think about the problem differently to come up with new solutions.
Basically it means that what you've/I've been thinking was the reason for something is not actually the original reason; we were thinking of wrong reasons.
