왜 제게 전화 안 하셨다? --> that sentence is in horrific form, so 어요 must not be omitted. The full sentence is supposed to be: 왜 제게 전화 안 하셨어요?

eg: 거의 다 했어: i have almost done.

I believe ~다 ending is only for declarative sentences, which makes them a bit formal.
You don't use it for interogative and imperative sentences, since there are other endings for those kind of sentences.
