    Which is it? is when you need to know which one you are referring to out of a selection

    What is it? is when you need to know what that something is

     

    Also some concrete examples to help out:
    a) Something has made you tired. What is it?
    b) You are tired because you ran really far or didn't sleep last night. Which is it?

    In the first example there are an uncountable number of things that could have made you tired. In the second example, the question is asking to choose between the two possibilities.
