    Here, رسیده means برسد

رسيده is a verb here not an adjective, and since the sentence tells about a happening in future you should translate it in future form.

Therefore the word رسيده in this sentence means "will arrive".

in future tense when we want to relate two or more sentences or to make some consecutive sentences we use PP(past participle) +"و" instead of "می رسد"or"خواهد رسید" and just conjugate the verb of the last sentence.

You can only use this structure in "declarative sentences" or 陈述句 ^_^
Here's the rule: If you have two verbs is a sentence which are of the same tense, you can simplify the first verb and let the second one come to it's help!!!

Let me give you some simple examples:

    Instead of saying:
من امروز به مدرسه می روم و درس می خوانم

    I choose to say:
من امروز به مدرسه رفته و درس می خوانم.

The first verb that I have changed is [رفته] and the second one is [می خوانم].
Both of those verbs are of the simple present, and I changed the first one to a present perfect. Look:
    می روم >>> رفته

    你明白了吗?^_^

    In your sentence:
پیش بینی می شود که این کاوشگر روز ششم دسامبر به نزدیکی کره ماه رسیده و وارد مدار ۱۰۰ کیلومتر دور کره ماه شود

    it was actually like this:
پیش بینی می شود که این کاوشگر روز ششم دسامبر به نزدیکی کره ماه برسد و وارد مدار ۱۰۰ کیلومتر دور کره ماه شود

Those two verbs i.e. [برسد] and [شود] are of a tense which is called [مضارع التزامی] in Farsi.
I think the first verb is always changed to a present perfect tense, exactly like [رسیده].

    再见!!! ^_^
