"Medieval" here means "very out of date" and"used in the Medieval period in history", roughly 1000 years ago and implies cruel principles then believed and implemented. The phrase says To see the killers simply as religious fanatics and not something more complex is doing what the group wants. The group believes the very old idea that people shouldn't be able to express freely their opinions and views and that idea should obviously be rejected.
