His pain was immediate and severe enough that it required intravenous sedation and pain medication. The nail was lodged (stuck) in the gap between his toes (or the gap of his toes), so it is saying there was a nail stuck either in his boot or in the gap of his toes, I believe.

It means the nail didn't get into any of his toes, but went through the gap between toes. So the nail did penetrate the boot, but didn't hurt him physically.
