Caretaker in the UK is someone who looks after the grounds of a building, usually a school. You are correct it isn't right for what you want. Care assistant is as a good a term as any. I don't think we really have a word for it, you would just say someone 'works in an old people's home' if you wanted to be more specific. The question should be what is the difference between an old people's home and a nursing home. A nursery is very different, nurseries are where young children go before they are ready to start school (guardería). An old people's home or retirement home is a type of care home where older people go to live sometimes because they would struggle to live alone. Its just to make their lives easier, they still have a fair amount of independence although obviously they are looked after by the people in charge of the home. However a nursing home is where the people have more serious illnesses where they cannot look after themselves e.g. they might have dementia or other degenerative illnesses, and they need special care.
