It comes from the Spanish for, 'hand to hand.' It is used to mean a fight on equal footing where neither participant is at a clear advantage. So in this context the officer is asking whether they got into a fight and were hurting one another or whether she was simply being beaten up by a much stronger, abusive boyfriend.
