"Thanks for checking in on me." What does it mean & how is it used?

The speaker is thanking someone for checking to see if they are okay. This might happen if a neighbor checked to see if an old woman had food, had enough heat on a winter day, had not fallen and hurt herself, etc. Then the old woman would thank the neighbor for "checking in on" her.
