"Forma" is a word used to describe usually a geometrical shape. In this case the shape is the one we use to call a loaf of bread. "Grossa" Is used as large. So the person wants a big loaf of bread to be bought.
Forma is also used for cheese like parmigiano and that's where the word "formaggio" comes from.
