Most likely, the surname Толстой comes from an adjective "толстый" (stout, fat). And it surely was the distinguishing feature of the noble family's ancestor. At the same time the nickname Толстый was not offensive in the Medieval Russia. It meant "a man full of health, strength and wealth"
