I think it's incorrect because "suggested" here is not a verb but a past participle functioned as an adjective.Let's have a look at another sentence: "The soldiers captured in the battle were sent to the detention camp." -> "captured" is a past participle, it's not a verb so we need "were" here. Therefore, I think the grammatical version would be: "Some causes have been identified and solutions have been suggested to deal with this issue." "suggested" is a past participle, not an adjective.

In this example, when one says "have been", it is not necessary to say the "have been" again before "suggested" as "have been" applies to both "identified" and "suggested" in this case. It is just a more concise way of writing the sentence. What you wrote is also correct, but as you said, sounds a little repetitive, so you can leave the second "have been" out.
