It's n opt something I've heard in England. We would be more inclined to say
It gives me the willies
or
It makes my skin crawl

I have heard it a few times in the US, both in the South and in PA (for what it's worth), more often than not used in a humorous way - sometimes use it myself precisely for that effect :). So, yeah, still in active use, albeit infrequent.

I would say (based on my opinion being the same as Peachey and Robert's) that it may be (still?) a bit more common in British flavours of English. I use it occasionally - I quite like the sound of it.

In American English, yes. It is not unusual. However I doubt it is used by everybody

It's not commonly used in the U.S. anymore. I've never used it. As Dan Smith said, you might hear people use it in a joking way(more so on TV, I'd say).
