When someone calls you and said “ are you busy ?” Can you respond as “ not at all” which means I am not busy.

Yes saying "not at all" is fine. But it would be a little informal like what you would say to a friend. At work you would likely say something like "No, not too busy. How can I help?"

Yes you can reply saying that, or “not really.”
