It means that you returned to some vice or bad behavior that you had earlier made a commitment to stop. Usually it refers to drinking.
The image is of a person riding a wagon full of other well-behaved people, then falling off and being left behind.
"Jim hadn't been drunk in two years, but he fell off the wagon at the New Year's party."
"I try to quit smoking, but I keep falling off the wagon."
