alla ( ends with a vowel) università (starts with a vowel) = all' università( delete the last vowel of "alla-" and add the apostrophe: it sounds better )
allo ospedale = all'ospedale

The same as before.
With articulated prepositions, when there is the apostrophe the "l" are always two.
all'università
all'ospedale
nell'auto
dall'auto
sull'albero
dell'uno e dell'altro
