What does "doctor" mean in this sentence? He's a doctor. Does it mean someone who treats people who are ill or someone who holds the highest level of degree given by a university?

Although it COULD mean both, if someone said that to me I would presume they meant a medical Doctor. If they were talking about any other doctor they would put that after. e.g. "He's a Doctor [of Mathematics]"

It means both: a holder of a doctoral degree and also it means to work as a doctor. Examples: Doctor Jones is a thoracic surgeon Dr. Molineu has a PhD in political science.
