The meanings are very similar. Evaluation is usually used to describe a detailed piece of work - for example a country might commission (hire somebody to do) an evaluation of its prison system. An assessment is often less detailed and can be completed more quickly. We also use the word assessment in connection with tests or exams but we don't use the word evaluation in this context.
