The context as below:
“How are you doing?” the nurses would ask me in melancholy tones.
“We’re holding up,” I’d say, as if I were a we.

"Holding up" is an idiom that means you are doing OK, even though circumstances are challenging. Example: You are sick. Somebody asks you how you are doing. You say "I"m holding up." It means even though I've got this sickness that is troubling me, the sickness has not defeated me and I'm still managing to be reasonably happy. A similar phrase is "I'm hanging in there."
