심하다 = too much, excessive - an action, situation, condition, and the like are out of the accepted norm.
심각하다 = serious, grave.
심하다 is a relative term, so it can be used lighthearted about small things as well as serious things (기침을 심하게 한다. 말이 너무 심했다. 심한 더위).
심각하다 is more absolute, usually reserved for something grave. (환경 문제가 심각하다. 회사가 재정적으로 심각한 상황에 처했다).
