If literal, then it means there is blood (actual blood) in their eyes.
If figurative, then yes, it means they are very angry. You may more commonly hear the saying They are seeing red to indicate anger.

Probably, but it depends on the context.
Their eyes are red, burning with anger, ready to kill.
