이름이 뭐예요? (common form; used in more of a casual setting and perhaps with someone closer to your age)
성함이 어떻게 되세요? (more polite form; used in a more formal setting and perhaps with someone who is noticeably older than yourself)
이름 = name (common form)
성함 = name (formal form)
뭐예요? = 'What is...?' (common form)

i'd say "이름이 어떻게 되요?".(nama sapa?)
this is more common and natural in spoken.
for formal, "성함이/이름이 어떻게 되세요?"(Siapa nama awak?)

There is a honorific in Korean.
When you ask a name in a honorific expression, It is the sentence "이름 좀 알려주세요~" (=Could you let me know your name?"
When you ask your firend's name "이름이 뭐야? "(What's your name)

In English : May I ask your name?
In Korean : 당신의 이름을 물어봐도 되겠습니까?(direct translation)
