Here, "terribly" simply means "very", "extremely". So she is very, very ill. Other examples of terribly=extremely are:
"He is terribly handsome."
"This dessert is terribly good."
"She works terribly hard."
These are all very positive statements. :)

It means that they feel very sick. They're not in good health right now.
