Hi Dariana, "outta" is a slang/shortened version of the two words - out of… "rehab", as Sal P pointed out, is short for - rehabilitation So, it means: "Right out of rehabilitation", which is to have just very recently left a rehabilitation centre, or rehabilitation program.

It means that person has left some rehabilitation center, which is usually a place where people go to recover from drug addiction.
