Which is the right "How are you" in Korean?

So as I'm learning some Korean Small Talk I stumbled across 2 different ways of saying "How are you?".

One of them is  지냈어요?  /  잘 지났어? and the other option is "어떻게 지내세요?  /  어떻게 지내? 

Is there any difference or can I just use both of them. Or is one of them politer than the other.

And if I get asked this question and answer it, how do I say "And you?" ? 

And could you also give me some examples for answers to this question. I'm good, I'm soso, I'm bad etc.

We use different greetings for different occasions.

1. first time encounter (introduction).

(more formal - preferred by males)
AA: 안녕하십니까? (Hello / How do you do?).  (저는) XX라고 합니다. (I'm XX) 
BB: 네, 안녕하세요. YY 입니다.

(female version)
AA: 안녕하세요.  XX라고 해요.
BB: 네, 안년하세요.  YY예요.

2. with someone we see regularly.
AA: 안녕하세요? (Hi / How are you?) or 안녕? (casual version).
BB: 네, 안녕하세요? (We tend to repeat the same phrase instead of "and you?")

3. with someone we see first time in a long time.
AA: (참 )오랜만이네요. (It's been quite a while since we met). (요즘) 잘 지내세요? (Is everything good with you?)
BB: 네, 잘 지내요(Yes, I'm fine). (XX 씨도) 잘 지내시죠? (Hope you're doing well  too)

어떻게 지내세요? and 어떠세요? are variations on 잘 지내세요?  (어떻게 = how, 잘 = well)
별일 없으시죠? (literally "I hope there isn't anything bad/unusual with you") is another one you can use.

괜찮아요 and 별일 없어요 can also be used for "I'm doing alright".

잘 지냈어요? -Have you been doing well? ----------semi formal/polite

잘 지내요? -Are you doing alright? ( how are you?)

어떻게 지내세요? - How are you doing?/ How's it going?  -------------------semi formal/ polite 

어떻게 지내? (Same as 어떻게 지내세요? informal/very casual.)

@Bonniesu Yeah thank you for translating! But so it doesn't matter which one I use right? (besides the honorifics)
