It works just like any other question. Even in past tense, the method is exactly the same.
1) Write the statement form of the question.
Ex.
You came here.
You went there.
He was happy.
2) If do/does/did is omitted (ie. if the first verb isn't an auxiliary verb), add it again.
Ex.
You did come here.
You did go there.
3) Switch the position of the subject and verb. If you want to ask a yes-no question, stop here.
Ex.
Did you come here?
Did you go there?
Was he happy?
4) If you want to use a question word (why, who, what etc.), write it at the beginning of the clause containing the question.
Ex.
How did you come here?
Why was he happy?
Why did you go there?

Generally they start with past tense , helping verb like, Did, had , have, For example, did you go there?, have you finished your lunch?, and had you been there?

Had you ever ridden a car like that before today?
Did you tell the truth while you were on the witness stand?
Were you really angry?
