Does the word "painly" exist, meaning "causing pain", like "dead" -> "deadly" Eg. "a thick painly truncheon" Or should I use painful instead? If "painly" is not a proper word, can I use it still in the quote marks to imitate child's speech? Thanks

No it is not a word. Plainly or painfully which have diffrent meanings. But not painly. It doesn't exist. Therefore I wouldn't use it at all.
