Could someone explain why 'breaks' is used instead of 'broke' for the blank 3 ? I know some might say it expresses a general truth or a state of mind rather than a specific past action. However, it feels to me like something that actually happened in the story as a past event—meaning it 'broke' the rule/balance at that specific time. Besides, state verbs like hate or like, which also describe mental states, usually follow tense agreement with the context (for example, 'He didn’t say anything, but he hated the guests wearing shoes inside'). Why doesn't that logic apply here?

It is absolutely a general truth. The em dashes indicate it is a parenthetical statement. That person who did not remove their shoes did not do so because it is a general truth that removing shoes breaks their rule.
