Output can happen constantly or just once. We can talk about the output of a factory - what it produces - throughout the year. An outcome is a one-time event. The outcome of a meeting might be a contract. The outcome of a war would be winners and losers (perhaps). The outcome of a soccer match would be the score or who won or lost.
