You can say:
- Pourriez-vous m'aider s'il vous plaît?, which means very politely "could you please help me?".
- Pouvez-vous m'aider? (could you help me?), also very formal, less polite than the former phrase.
Or simply:
- Peux-tu m'aider? (can you help me?)
