The nail was lodged (stuck) in the gap between his toes (or the gap of his toes), so it is saying there was a nail stuck either in his boot or in the gap of his toes, I believe.

It means the nail didn't get into any of his toes, but went through the gap between toes. So the nail did penetrate the boot, but didn't hurt him physically.
