Hello! My question is what is the correct order when I use frequency adverb in a question with "what, when, why" and "do, does"

For example is "What do you usually wear to work?" or "What usually do you wear to work?"
"Why does she always wear that dress?" or "What always does she wear that dress?"

I put the frequency adverb before the auxiliary verb (do/does) or after?

The adverb of frequency always goes before the main verb:

What do you usually wear to work?
Why does she always wear that dress?

The only exception is when the verb is "to be":

Why are you always late?
Is she usually a nice person?
