They all convey the similar meaning: all/everyone, but have slight differences.
다 or 모두 usually means "all"
여러분 usually means "everyone" "Ladies and Gentlemen"
Notice "all" can refer to either a "subject" or "people", while "everyone" can only be referred to "people." In the same token, 모두 can describe either a "subject" or "people", while 여러분 can only describe "people."
For examples,
이거 모두 / 다 / 모두 다 가짜다. They are all fake.
모두 / 다 / 모두 다 행복하시길 바랍니다. I wish you all to be happy.

모두 and 다 can be used interchangeably in most cases. When they are used together (as 모두 다), it's simply emphasizing the meaning "all" in a majority of cases. For example,
모두/다/모두 다 멋집니다. This can all mean either "Somethings are awesome" or "All of you are awesome."
