He is referring to the metaphor to 'wear one's heart on one's sleeve' which means to share one's feelings. I think what the singer is saying is that he will continue to share his feelings (through his music) until his 'sleeves are stained red with the blood from the heart that he wears on his sleeve' (metaphorically). Does that make sense?
