This is one of those Tagalog words/phrases/sentences that have no direct English translation ( the phrase "ika-ilan," in particular), but it means something like "Where do you place in you and your siblings' birth order? (are you the firstborn child, the second one, the third one, and so on?).
"na anak" in this context can roughly translate to "the child that is."
E.g.
Siya ang panganay na anak. = He or she is the firstborn child / He or she is the child that is the firstborn.
