I grew up in Colorado and we usually say: "I'll have a..." so you can say: "I'll have a..." cheeseburger/hamburger/fries/coke/etc/ "Can I get a..." cheeseburger/hamburger/fries/coke/etc/ "I would like a..." cheeseburger/hamburger/fries/coke/etc/ "I'll take a..." cheeseburger/hamburger/fries/coke/etc/ since you mentioned In-N-Out you could say: "I'll have a number 2, with no onions, extra tomatoes and light sauce and a Coke." hope that helps! :)

If you are in the restaurant but want a take away, then: " Hello.
