Well done. This was a good answer written with the right formal tone. You covered the requirements of the task. In the UK if you start a letter Dear Sir/Madam you end Yours faithfully. If you know the name - Dear Mrs Smith then you end Yours sincerely. Actually I find this question a bit strange. The only problem is that the company sent you something you didn't want or pay for. This is their problem! I would write the final paragraph like this: While I am happy to keep the purple t-shirt as a gesture of goodwill from yourselves, I am prepared to return it if necessary. If so, please send me the appropriate packaging and postage label and I will arrange for its return at my convenience. I haven't taught General IELTS so I am not totally aware of the requirements. I will check!
