1. The cat I take care with yellow eyes is sleeping.
2. The cat with yellow eyes I take care is sleeping.
They’re both “correct”, but the second one is far clearer and more natural.

As a native English, niether sentance sounds completely natural. I recommend you add the word "of" to the first sentence and the commas. The rule is: you need to be able to read the sentence with the phrase between the commas removed. In this case it would read "the cat i take care of is sleeping". Then you can add in the phrase "with yellow eyes" to give more detail.
