Phrase "I will not fail you again" I always hear this phrase "I will not fail you again" in movies, sermons or quotes. I have a feeling that it has a very deep meaning or history. Can anyone explains what does it means , when should we use it and why?

The only real nuance of the phrase is that it's formally written (no contractions). So this kind of promise seems like a binding obligation.

We understand "fail you" in this sentence as "fail in my service to you". So there's also the master-servant dynamic in this phrase.

That's about the size of it, really. :)
