Which one to choose Hello there

Which one of the following sentences is correct !? 

A. May I ask what your name is !? 
B. May I ask what is your name !? 
Or
C. Could you tell me where you are from !? 
D. Could you tell me where are you from !? 

I know that we shouldn't use double questions so the correct answers might be A and C but the other two options seems pretty correct to me.
I think they may be wrong gramatically but correct in spoken.

And about the above question, should I say "which ones of the following sentences are correct?" or "which one of the following sentences is correct?"

You are right, A and C are correct. B and D form double questions, and they would require a comma to separate the clauses.

e.g. "May I ask, what is your name." 


"Which one of the following sentences is correct?" would be the correct way to phrase your question. 

However, "Which of the following sentences are correct?" would be a better way to phrase your question, as two sentences are right. (A and C)
